Job Description:

ComfortCare Pharmaceuticals is seeking a dedicated and technically sound Community Health Extension Worker (CHEW) to join our clinical team. In the community pharmacy space, the CHEW is the vital link between pharmaceutical care and preventive health. This role is designed for a "Wellness Facilitator", a professional who can blend clinical diagnostic skills with the empathy required to educate and guide patients toward better health outcomes.

Your mission is to ensure "Patient Safety and Proactive Health Management." You will be responsible for the foundational layers of our clinical services: from conducting vital sign assessments and supporting immunization programs to managing chronic disease monitoring for conditions like hypertension and diabetes. If you possess "Clinical Grit" and a deep understanding of medication adherence, we want you to help us deliver quality healthcare.

Requirements:

1. Diagnostic Assessment & Chronic Care Monitoring

• Vital Sign Management: Conduct precise patient health assessments, including blood pressure, blood glucose, temperature, and weight monitoring, to identify health risks early.

• Chronic Disease Surveillance: Support specialized monitoring programs for patients with hypertension, diabetes, and arthritis, ensuring their conditions are tracked and managed effectively.

• Clinical Documentation: Maintain meticulous and audit-ready patient records, documenting every clinical interaction to ensure continuity of care.

2. Immunization & Medication Support

• Supervised Immunization: Assist in the delivery of vaccination services under the direct supervision of the Pharmacist Manager, ensuring strict adherence to cold chain and safety protocols.

• Dispensing Synergy: Support pharmacists in medication dispensing activities, providing a second layer of verification and patient support.

• Pharmacovigilance Observation: Proactively observe and report any adverse medication reactions or patient concerns to the supervising pharmacist to ensure immediate intervention.

3. Health Education & Community Outreach

• Patient Counseling: Provide authoritative guidance on disease prevention, healthy lifestyle choices, and the critical importance of medication adherence.

• Wellness Promotion: Actively promote the pharmacy’s preventive care initiatives and wellness programs to the local community to drive health awareness.

• SOP Adherence: Maintain the highest standards of ethics and professionalism, ensuring that all clinical service areas are organized, sterile, and compliant with ComfortCare SOPs.

Qualifications and Skills:

Professional Profile

• Education: Diploma or Certification as a Community Health Extension Worker (CHEW) from a recognized health technology institution.

• Licensing: Possession of a valid professional license or registration.

• Pharmacological Knowledge: Good understanding of common medications, including their indications, basic dosing, and critical patient counseling points.
