Job Description:
We are seeking a dedicated and compassionate Linkage Coordinator to bridge the gap between community-based HIV testing and long-term facility-based care. You will be the primary advocate for people living with HIV (PLHIV), ensuring that every newly identified individual is successfully enrolled in treatment, supported in their adherence, and retained within the continuum of care. This role is essential for our mission to strengthen community support systems and improve health outcomes through strategic coordination and collaboration.
Requirements:
Referral & Linkage Strategy: Develop and monitor HIV referral systems between health facilities and communities. You will track outcomes to ensure "no patient is left behind" once identified.
Client Support & Retention: Provide direct support to newly diagnosed PLHIV, assessing barriers to care and facilitating successful enrollment. You will oversee follow-up mechanisms (home visits, calls, messaging) to ensure patient retention.
Community Partnership: Collaborate with CBOs, FBOs, NEPHWAN, and local structures (WDC, VHW) to foster synergy and integrated care. You will establish and facilitate support groups and lead community adherence monitoring.
PHDP Implementation: Lead the implementation of Positive Health, Dignity, and Prevention (PHDP) initiatives at the community level.
Capacity Building & Data: Supervise data collection processes to ensure accuracy and provide training/capacity building for healthcare workers and CBO partners on referral and linkage protocols.
Qualifications and Skills:
Education: BSc, BA, or equivalent relevant qualification.
Experience: 2–3 years of experience in providing HIV-related services within both community and clinical settings. Experience managing CBO partners is a significant plus.
Language & Communication:
Fluency in English with strong writing skills for reports and technical documentation.
Ability to communicate effectively with patients in a sensitive, empathetic, and professional healthcare environment.
Exceptional customer service skills for engaging with clients and regulatory stakeholders.
