Job Description:

We are seeking a dedicated and detail-oriented Clinical Pharmacist to join our multidisciplinary healthcare team. In this role, you will be responsible for ensuring the highest standards of pharmaceutical care through prescription validation, medication therapy monitoring, and patient counseling.

You will work closely with physicians and nursing staff to optimize therapeutic outcomes and ensure the safe, ethical, and efficient dispensing of medications within the hospital.

Requirements:

Clinical Pharmacy & Patient Care

• Prescription Validation: Review and validate prescriptions to ensure clinical appropriateness, safety, and dosage accuracy.

• Therapy Monitoring: Monitor patient drug therapy and provide evidence-based recommendations for dosage adjustments or alternative treatments.

• Patient Counseling: Educate patients on medication adherence, proper administration techniques, and potential side effects to improve health outcomes.

• Collaborative Practice: Participate in ward rounds and clinical meetings to provide expert pharmacological insights to the medical team.

Pharmacy Operations & Compliance

• Regulatory Adherence: Ensure all pharmacy activities comply with the Pharmacists Council of Nigeria (PCN) guidelines, legal statutes, and hospital policies.

• Inventory Management: Oversee the storage, dispensing, and documentation of all medications, ensuring the availability of essential drugs.

• Staff Supervision: Provide leadership and guidance to pharmacy technicians and support staff to maintain operational excellence.

• Documentation: Maintain meticulous records of drug interventions, controlled substances, and general pharmacy transactions.

Qualifications and Skills:

• Education: Bachelor of Pharmacy (B.Pharm) or Doctor of Pharmacy (Pharm.D) from an accredited institution.

• Registration: Must be a Fully Registered Pharmacist with the Pharmacists Council of Nigeria (PCN).

• Licensing: Possession of a currently valid practicing license.

• Experience: Minimum of 2 years of clinical experience in a hospital setting is preferred.

• Skills: * Strong interpersonal and communication skills for effective patient counseling.

  • Ability to thrive in a multidisciplinary, fast-paced medical environment.

  • High level of professional integrity and clinical judgment.
