Job Description:
We are seeking a clinically-minded Administrator / Care Assessor to join our leading health and social care team. This is a critical dual-function role: you will serve as the architect of personalized care plans for new patients while maintaining the administrative backbone of our quality assurance systems. You will be responsible for ensuring that every patient’s strengths and preferences are translated into a safe, compliant, and actionable care strategy.
Note: This is initially a temporary role to provide coverage during the Christmas period, with a significant opportunity to transition into a permanent position based on performance and business needs.
Requirements:
1. Patient Assessment & Care Architecture
Comprehensive Evaluation: Conduct detailed assessments for new patients, identifying personal preferences, physical needs, and individual strengths.
Dynamic Care Planning: Design and update personalized care plans and risk assessments, ensuring they evolve in real-time as a patient’s condition or circumstances change.
Clinical Review: Rigorously review visit notes and medication records (MAR charts) to identify gaps in care or potential medical risks.
2. Compliance & Incident Governance
Quality Assurance: Compile records and detailed reports necessary to maintain strict adherence to care quality and regulatory compliance standards.
Incident Management: Assist in the creation and management of incident reports, identifying root causes and recommending preventative measures.
Administrative Synergy: Provide ad-hoc administrative support to ensure the smooth day-to-day operation of our healthcare business.
Qualifications and Skills:
Professional Profile
Experience: Proven experience in the medical field is highly preferred.
Analytical Power: Essential ability to solve complex care challenges and translate clinical data into effective care solutions.
Communication: Exceptional proficiency in both written and verbal English.
Independence: Ability to thrive in a fast-paced environment with minimal supervision while maintaining a collaborative spirit.
Home-Office Technical Specifications (Mandatory)
Hardware: A personal laptop with high performance (capable of handling multiple professional applications simultaneously).
Environment: Access to a reliable power supply (Inverter/UPS/Generator) to ensure zero disruptions during 9 AM – 6 PM shifts.
Connectivity: A stable, high-speed internet connection capable of supporting seamless video conferencing and large data transfers.
Digital Literacy: Proficiency in Windows, Microsoft Excel, and PowerPoint.
